modified vehicle report form for Adrian Flux Insurance

Having just restored my Bus ,I have been asked to provide a Modified vehicle report form ??
does anyone know where i am supposed to get this from or where i need to go

They used to send them out with the policy.
It's a couple of pages but nothing too challenging.
Main reason I try and avoid Flux these days is the endless paperwork but they're one of the few who will give a sensible quote on the beetle.... :roll:
