V55/5 for 69 Bay

Finally managed to get my Bay to MOT to start the registration process and received the V55/5 in the post to complete.

Has anyone done one for a 69 bay ? I need the technical details info and would appreciate if someone has an example (personal details deleted) as the previous countries documents don't have the same details.
